Crossett is located in Ashley County, Arkansas in the southern part of the state about 133 miles southeast of Little Rock and about 89 miles south of Pine Bluff.

The fatal crash occurred at 2:32 p.m. in the afternoon when a 2005 Pontiac with 3 occupants was traveling north on Highway 133 and pulled out in front of a southbound 1998 Peterbilt semi truck.

The victim was 16 year-old Alex Taylor, a passenger in the car.

The driver of the Pontiac was 16 year-old Peyton Bannister, who tried to make a left turn onto King Circle from Highway 133 when the crash occurred. She was seriously injured in the accident. The other passenger was 16 year-old Robert Burton, who was also injured.

The Ashley County Ledger reports:

Bannister was transported to Jefferson Regional Medical Center in Pine Bluff where she underwent surgery. As of Thursday afternoon, she remained in intensive care. Burton was treated at the Ashley County Medical Center and released.

According to the Arkansas State Police crash record, the driver and the passenger who survived were wearing their seat belts. Taylor was not wearing his seat belt.

The big rig driver, Eric A. Tollefson 40 years old, from Camden, AR, was not injured.

The Police report indicated that it was clear and dry at the time of the accident, so weather did not appear to be a factor.

The National Transportation Safety Board (NTSB)

According to Deborah A.P. Hersman, chairman of the National Transportation Safety Board, fatigue “is one of the most insidious issues in the transportation industry,” and more must be done to fight it, she said.

“We establish a 72-hour prior history in every NTSB investigation,” Hersman said at the National Press Club in Washington Nov. 16. “Unfortunately, we find fatigue in more incidents and accidents than you would think.”

She said the NTSB has recommended the Federal Aviation Administration set hours of service rules for flight crews, aviation mechanics and air traffic controllers, stating it took Congress to change century-old rail employee hours of service rules.

The Federal Motor Carrier Safety Administration, which has issued truck driver hours of service rules three times since 2003, plans to propose a new HOS rule early next year. Read more.

Fayetteville, NC Tanker Crash

The tanker truck Graves was driving caught fire after he struck a guard rail and punctured his fuel tank. The truck accident was part of a chain-reaction crash.

In the accident Graves clothes caught on fire, then he jumped from the cab.
