On Wednesday night the driver of a semi trailer truck was headed northbound on Interstate 495, moving through Essex County, MA. The driver was identified as 53 year old Alan Streelow of Brunswick, Maine. It is not known what the semi truck was hauling at the time of the truck accident but Mr. Sweelow confirmed that he was headed to Portland, Maine.

Shortly before 8:00 p.m. Wednesday night the truck driver was going through the far north Massachusetts city of Amesbury. The truck accident occurred as the 18 wheeler was approaching the highway bridge that passes over Route 110, near the interstate’s merger with Interstate 95. According to the truck driver a wheel on his semi trailer blew out and he lost control of the tractor trailer. The truck driver fought to keep his tractor trailer on the interstate but was unable to regain control and the semi jackknifed. He crashed into the guardrail running along the side of the overpass bridge.

The semi trailer finally came to a rest with the cab section hanging on the edge of the guardrail, dangling over the Route 110 traffic lanes below. About 200 feet of guardrail was damaged in the truck accident. Several parts of the destroyed guardrail fell onto the Route 110 traffic lanes but fortunately no one was hit by the falling debris.

No other persons or vehicles were involved in the truck accident.

Police and emergency personnel responded to the truck accident and helped the driver, Mr. Sweelow, escape from the cab suspended over Route 110. He received minor injuries in the crash but it is not believed that he required hospitalization.

The force of the crash punctured one of the semi truck’s gas tanks and diesel fuel began to spill out onto the northbound lanes of I-495. Highway crews responded to the truck accident site and began to spread sand over the approximately 130 gallons of fuel that spilled on the road surface. As the crews worked through the night snow began to fall, making their work to prepare the interstate for the Thursday morning commute more difficult.

Massachusetts State Police cordoned off a portion of the highway around the crashed semi truck so workers could remove the disabled vehicle. Once the 18 wheeler was pulled off the guardrail it was towed from the truck accident site. Drivers on northbound I-495 were forced into one lane as they passed the crash site. The lane reduction would last into the morning commute, causing traffic delays.

MA State Police reconstructed the truck accident. Early reports stated that the truck accident happened when the driver swerved to avoid hitting a spare tire that flew off another vehicle. The truck driver, Mr. Sweelow, later told officials that it was a tire on his semi that blew out and he lost control of the rig and it jackknifed. The tractor trailer would be inspected to see if there were any mechanical failures that led to the crash.

According to some reports the truck driver may not be charged in the Wednesday night crash. The truck accident investigation is still open.

Monday afternoon a 45 year old truck driver from New York was driving a semi trailer through Norfolk County, MA. The trailer section of the semi tractor truck was filled with a cargo of candles and it is believed that the semi is owned by a trucking company based in Philadelphia, Pennsylvania. The identity of the semi driver and his destination has not been released at this time.

The truck accident occurred shortly after noon on Monday as the semi trailer truck was moving south on Interstate 495. The driver was just south of the city of Foxboro and was attempting to get on southbound Interstate 95. As the truck driver went around the curve of the ramp he suddenly lost control of his 18 wheeler rig for unknown reasons. The tractor trailer apparently lost its balance and flipped over onto its left side, trapping the driver inside the cab section. The semi came to a rest across the ramp, blocking the traffic lane.

No other persons or vehicles were involved in the truck accident.

A passerby witnessed the rollover truck accident and stopped to give aid to the driver. The passerby, who was not identified, helped to free the driver from his overturned truck. Emergency personnel from the cities of Foxboro and Mansfield quickly arrived at the crash site and treated the driver, who had suffered serious injuries to his head and other parts of his body.

Once the driver was stabilized he was airlifted from the truck accident scene by a Boston Med Flight medical helicopter to Brigham and Women’s Hospital in Boston. His injuries are considered potentially life threatening and he remains in critical condition.

Foxboro firefighters and police officers also responded to the site of the roll-over truck accident. The trailer of the semi remained intact during the crash and the cargo of candles did not fall out on the highway. There were also no reports of diesel fuel spilling out from the gas tanks.

Police shut down the section of the I-495 ramp to southbound I-95 while highway crews worked to clear the lanes. The semi truck was dragged off the ramp and onto a grassy area next to the interstate where workers transferred the load of candles onto a second tractor trailer. Once the disabled semi was empty a heavy duty tow truck hauled it off the interstate. The lane closures would last for most of the afternoon, impacting the afternoon commuter rush.

Authorities conducted a reconstruction of the truck accident but have not yet released their initial findings to the public. The area of the crash has had many rollover incidents in the past and it may be that the design of the off ramp’s curve is too sharp for large 18 wheelers to maneuver safely.

At this time the driver has not been cited and the truck accident remains under investigation.

After the truck accident police shut down Cottage Street between Clark and Pine Streets, and Chapel Street was also closed for the rest of the day while highway crews cleared the downed poles from the roadway and the Union Mart store.

At around noon on Friday, July 16, the truck driver was traveling on westbound State Route 141 toward the downtown section of Easthampton when the driver turned onto Chapel Street and attempted to make a U-Turn. While making the turn at the juncture of Chapel and Cottage Street the 18 wheeler truck hit the line or guy wire on a utility pole.

According to witnesses, the truck driver apparently did not realize the truck had hit a wire and the driver continued to travel down the road uprooting the utility pole that would fall on the Union Mart store. The semi truck also pulled down an electrical transformer on Chapel Street and cut off the top section of another utility pole at the corner of Chapel and Maple.

The truck accident also severed the service lines to the residential homes on Gaston and Maple Streets. A total of five utility lines were taken down in just a few minutes by the semi trailer truck.

After causing the destruction, which authorities say covered over a hundred yards and closed down several streets, the truck driver did not stop. Officials say that the semi truck was last seen getting back onto Route 141 and headed toward the city of Holyoke.

After the truck accident police shut down Cottage Street between Clark and Pine Streets, and Chapel Street was also closed for the rest of the day while highway crews cleared the downed poles from the roadway and the Union Mart store. Utility personnel worked to restore power to the area.

According to Western Massachusetts Electric Company 103 customers lost power due to the truck accident.

The unknown truck driver will likely face several serious charges when found. Some investigators of the truck accident believe the driver should have heard the large poles snapping and realized that the 18 wheeler truck was hitting something. The only clue that they have is that the semi truck is red or blue and had the initials “AL”. They also expect that it will have damage to the top section where it took out the utility poles.

The truck accident remains under investigation.

Firefighters worked to contain the jet fuel spill using foam, dry ice, and other materials.

Shortly after 4:30 a.m. Sunday morning, 36 year old Richard Williams of Manchester, New Hampshire was driving a 2006 FrueHauf semi tanker truck south on Interstate 95. The tanker was carrying thousands of gallons of jet fuel and the driver was on his way to deliver the fuel to T.F. Green International Airport in Warwick, Rhode Island. The tanker truck is owned by C. White and Sons, Inc, a petroleum transport company based in Rocky Hill, Connecticut.

As Mr. Williams drove through Norfolk County near Foxboro, he apparently lost control of the 18 wheeler as he was trying to stop the truck. The tanker truck rolled over and crashed on Interstate 95 at the off ramp for northbound Route 495. The impact of the truck accident damaged the tanker and about 11,000 gallons of the jet fuel spilled out onto the highway. No other vehicles were involved in the crash.

The Massachusetts Department of Environmental Protection sent technicians to the truck accident site to aid the local firefighters in the clean up.

The Foxboro Fire department and emergency personnel responded to the site of the truck accident. The truck driver was treated and stabilized at the scene and then transported to Rhode Island Hospital in Providence, RI. His injuries were reportedly minor and not life threatening.

Firefighters worked to contain the jet fuel spill using a combination of foam, dry ice, and other materials. The Massachusetts Department of Environmental Protection sent technicians to the truck accident site to aid the local firefighters in the clean up.

The tanker company, C. White and Sons Inc, also contracted with a private environmental recovery company to send a Haz-mat team to help clean up the jet fuel spill. According to company spokesman, the clean-up and recovery was expected to last about three days while teams vacuumed up the spill, treated any ground that was contaminated by the fuel, and ensured that any nearby drainage or water systems were clean and safe.

Authorities were also at the scene to reconstruct the truck accident and determine a cause for the rollover.

MA State Police shut down a section of the interstate around the truck accident site including both southbound and northbound lanes. The closures would last for more than seven hours. All lanes were finally opened shortly before 7:30 p.m. Sunday evening.

At this time no charges have been brought against the driver and the truck accident remains under investigations.

Shortly after 1:00 p.m. Tuesday afternoon, 50 year old Douglas Mace of Waltham was driving a Boston Bark semi tractor trailer loaded with woodchips and light yard waste north on US 95/Route 128 in Waltham, MA. Boston Bark is a landscaping business based in Waltham.

The truck driver was reportedly speeding when he suddenly lost control of the 18 wheeler in the area near Exit 26 and the entry ramp from westbound Route 20. The trailer truck tipped over on its side, skidded for a short distance, and finally came to a stop blocking several lanes of traffic.

The rollover truck accident caused major delays on US 95/Route 128 and other nearby roads as officers had to shut down the area around the disabled truck while crews were brought in to clean up the traffic lanes and move the trailer truck off the highway.

No other drivers or vehicles were involved in the truck accident.

Emergency personnel responded to the scene of the rollover and treated the truck driver before transporting him to Lahey Clinic Medical Center in Burlington, about 12 miles away. Authorities did not release the details of the truck driver’s injuries but they are believed to be non life threatening.

MSP closed three lanes of traffic on US 95/Route 128 and the ramp to westbound Route 20, and traffic was forced to use one lane for several hours. These closures backed up traffic to Route 9 in Wellesley for over an hour. The highway was finally reopened shortly before 5:00 p.m. Tuesday during the afternoon commuter rush.

Vincent Mula, the owner of Boston Bark and the semi trailer involved in the truck accident, issued a statement that the company considered Mr. Mace “a very good driver, a very safe driver.” Mr. Mula commented that the truck driver may have been cut off by another vehicle or lost control when he was trying to get off the highway.

Massachusetts State Police officers also responded to the truck accident to investigate and determined that the truck driver was at fault. MSP issued Mr. Mace a citation for speeding.

The 18 wheeler ended up on its side, partially in a gully at the side of the interstate. The tractor trailer finally stopped, blocking two lanes of traffic and trapping Mr. Ramos in the cab of his truck.

The truck accident happened at approximately 3:30 p.m. Tuesday afternoon as an unidentified 17 year old boy and his passenger, 47 year old Larry Armstrong, both of Attleboro, MA, were in a 1997 Chevrolet Geo Prism. They were entering the southbound lanes of Interstate 495 from the exit ramp of Interstate 93 North.

After the teenage driver entered the interstate he attempted to merge into the center lane and lost control of the Geo Prism. The car smashed into the guardrail and then spun back into the interstate lanes directly in the path of a southbound trailer truck driven by 33 year old Varry Ramos of Worcester, MA.

The truck driver was unable to avoid an accident and crashed into the Geo Prism. After striking the car, the tractor trailer then went out of control and rolled over on the interstate’s right shoulder. The 18 wheeler ended up on its side, partially in a gully at the side of the interstate. The tractor trailer finally stopped, blocking two lanes of traffic and trapping Mr. Ramos in the cab of his truck.

Andover firefighters and emergency personnel responded to the truck accident. The 17 year old driver and his passenger were able to get themselves out of the smashed up Geo Prism. They were treated and stabilized at the scene and transported by ambulance to Lawrence General Hospital. They both reportedly suffered serious but not life threatening injuries.

Andover firefighters worked for about an hour to extricate Mr. Ramos from the trailer truck cab, using a hydraulic device to get the truck driver out. Once free Mr. Ramos was stabilized and then taken by medical helicopter to Beth Deaconess Hospital in Boston, MA. The latest report listed Mr. Ramos as being in fair condition suffering from serious injuries.

After the truck accident, Massachusetts State Police shut down all southbound lanes of I-495 while crews removed the wreckage from the area. A flatbed truck was brought in to remove the remains of the crushed Geo Prism. A team from Coady’s Towing in Lawrence, MA used a heavy duty crane to hoist up the overturned trailer truck back up on its wheels. The 18 wheeler was then removed from the truck accident scene.

The truck accident also caused about 150 gallons of diesel fuel to spill out of the truck. A crew from the State Department of Environmental Protection was called to the scene to contain and clean up the fuel spill.

The southbound lanes were closed for over three hours, reopening around 7:00 p.m.

At this time no charges have been bought, pending the completion of the truck accident.

Other departments working on the investigation included Mass Highway, State Police Crime Scene Services Station, Collision Analysis Reconstruction Section, and the Commercial Vehicle Enforcement Section.

Truck smashed through the white picket fence and slammed into a house at 498 Lakeview Ave.

At around 10:30 a.m. Saturday morning, 59 year old Eric Gremm of Tyngsboro, MA, was driving a flatbed truck, delivering lumber for Sherburne Lumber, based in Tyngsboro….

The truck driver, Gremm, was eating a small chili he had purchased from Wendy’s as he drove down Lakeview Avenue in Lowell. At one point the flatbed truck hit a bump in the road and according to Gremm the chili “went down the wrong pipe”.

Gremm began to choke and cough. The last thing he remembers is feeling light-headed, and then he reportedly lost consciousness. The truck driver lost control of the vehicle and the flatbed trailer truck left the roadway. It smashed through the white picket fence and slammed into a house at 498 Lakeview Ave. The truck accident created a large gaping hole in the home and knocked the structure about a foot off its foundation.

Local emergency personnel responded to the scene and treated the truck driver. He reportedly told the paramedics that he remembers choking on the chili, but did not remember passing out. Gremm was then transported to Lowell General Hospital. There were no reports on his condition, but the bridge of his nose was injured in the truck accident.

The first floor resident of the duplex house, Anne Marie Giaquinto, was home from work at the time of the truck accident, but luckily was in another room when the flatbed truck came crashing into the house. She was uninjured but shaken by the truck accident and was taken to the hospital to be checked out.

Maria Anstiss, who owns the home and lives on the second floor with her family, was not at home at the time of the truck collision. She told local media that she was glad, because “It could have been devastating for my kids”.

A nearby neighbor, Angel Cruz, said he felt his own house shake and when he looked he saw the first floor occupant, Ms. Giaquinto, trying to jump out the window. He said he went over and saw the injured truck driver.

The flatbed truck’s front wheels crashed into the house and through the first floor, into the basement. A main support beam, which is now resting on the furnace, is the only thing preventing the flatbed truck from sliding into the hole.

Because of the amount of structural damage sustained in the truck accident Lowell city officials believe it could be unsafe, and made the residents move out temporarily. They were allowed to return for a few minutes to gather belongings. Officials are leaving the truck in the house to keep the structure from collapsing until a structural engineer can come out. Utility workers shut off the gas and electricity to the house.

Structural engineers came out and examined the house on Monday. The city’s acting building commissioner, David St. Hilaire, said that the engineers have determined that the house is destroyed and will have to be demolished. The truck will have to remain in the house until it is demolished, because the engineers feel if it is removed first the structure could collapse and damage nearby houses.

The owners, Jerry and Maria Anstiss, salvaged what they could from their home.

Lowell Police officers responded to the truck accident scene, as did the Massachusetts State Police. No charges have been made at this time, but the truck driver could be cited for eating while driving. Police explained that even eating can be a potentially dangerous driving distraction.

The truck accident remains under investigation.

Troopers said it appeared that the semi truck skidded on black ice, hitting a 2003 Chevy Impala that was driven by a 65-year-old woman. The woman was not injured in the collision. The tanker was empty so there was no spill on the roadway, however, the accident caused the semi tanker to separate from the cab.

Troopers said the driver, Jamal Davis, 35, of Danville, N.H., was trapped inside the vehicle and emergency crews had to work to extricate him. They were able to get him out of the cab by 7:30 a.m. He was conscious and was taken to the Lahey Clinic in serious condition.

Troopers cautioned drivers to slow speeds when driving in areas where there is black ice.

“The temperature did drop overnight so we do see black ice occasionally. We want everyone to wear their seat belt, follow the speed limits, just stay within the rules of the road,” Trooper Thomas Murphy said.

Officers said the accident is still under investigation. The driver has not been cited yet, but may be once the investigation is completed.

This is not the first time that Abenaqui Carriers of North Hampton, N.H., has been involved in a major truck accident.

The New Hampshire based trucking company has issued a statement on their website saying, “The safety of our operations is our top priority and we will work directly with the authorities as they conduct their accident investigation. We apologize for any inconvenience this caused morning commuters.”

Two tractor trailer rigs on their sides at the Weston toll plaza on the Mass Turnpike

The truck accident occurred when the rig carrying the load of beef struck the tractor trailer carrying the styrofoam cups, invloving the passenger car – a Toyota Corolla – that was crushed in between the two rigs. Both semi trucks tipped over on their sides, one on the left side and one on the right side spilling beef carcasses all over the roadway and causing a major traffic jam at the Weston toll plaza.

The driver of the passenger car suffered minor facial injuries and was taken to a local hospital. No other injuries were reported. Watch video from Fox channel 25, Boston:

Police are still trying to sort all of this out, but the driver from Oklahoma says he was driving the appropriate speed when he was rear-ended by the other truck from Michigan who was carrying the load of beef. The truck driver from Oklahoma, said, “I was coming into the toll booth … at the regular speed you’re supposed to with E-ZPass, and all of a sudden I saw out of the corner of my eye this truck coming and he wasn’t stopping and then he plowed right into me.”

The collision caused a major traffic jam at the toll plaza so Police were basically waving all the traffic through these two lanes to try to ease up some of the traffic congestion but the problem is that it was a huge mess and cars were snaking their way through two open lanes of the toll plaza.

There were three different operations underway involving State Police. State Troopers were trying to get some of the traffic through the toll plaza through just two of the lanes that were open at the time.

The other thing that was going on was that there were tow truck operators who were working to try to get some of this wreckage out of there and then of course cleanup the mess. There were these beef carcasses from one of the trucks strewn across several of the lanes on the Pike.

The third thing is that there is an investigation underway by the State Police who are trying to figure out why all this happened in the first place. They are trying to understand whether it was speed or whether it was a mechanical problem, or perhaps if one of the drivers fell asleep.

The scene of the truck accident was cleared up by about 10:30 am, but the accident is still under investigation.
