Bristol is located in Elkhart County, Indiana in the northern part of the state about 201 miles northeast of Bloomington, 73 miles northwest of Fort Wayne and 90 miles northeast of Gary.

The accident happened around 7:45 p.m. at about mile marker 103 on the Indiana East-West Toll Road. Reports state that the minivan was traveling eastbound with 10 people inside when it hit a deer and then was rear-ended by the semi truck.

The area of the accident location is bordered by County Rd. 4 on the north, East Vistula Street on the south (Indiana State Road 120), County Rd. 129 on the west and County Rd. 131 on the east.

Four others were injured in the accident – three from the minivan plus the semi truck driver. Two of the most severely injured victims were airlifted by Life Alert helicopter and two others by ground ambulance to nearby hospitals.

According to reports, when the minivan hit the deer, the lights went out. It is quite possible that when the minivan’s lights went out, the big rig driver could not see it until it was too late.

By the force of the impact, both the minivan and the 18 wheeler went into the median after the collision.

The toll road was shut down in both directions and traffic was backed up for miles as police, rescue workers and fireman worked the scene of the accident. Eastbound traffic was diverted at mile marker 96 and westbound traffic was diverted at mile marker 107.

According to an Indiana State Police official, the trucker was tested and found that there were no drugs or alcohol in his system. He was treated and released from the hospital.

There are no details on the victims from the minivan, although it was reported that they were from Ecuador. Serious accidents of this nature often result in brain injury or death. The vehicle was licensed out of Illinois. The tractor-trailer was licensed out of Wisconsin and the trucker is from Rhode Island.

The accident occurred at about 1 p.m. at the Sinclair and Carroll Streets railroad crossing. The train was unable to stop in time to avoid crashing into the 18 wheeler that was stuck on the tracks, police told reporters.

The impact severed the semi to leave the cab on one side of the tracks and the trailer on the other.

There did not appear to be any injuries at the time of the accident.

Emergency crews were called to the scene to lift the tractor trailer so the train cars could continue moving.

The incident is under investigation at this time.

Early Thursday morning 54 year old truck driver John E. David of Huber Heights, Ohio was driving a semi trailer truck west on US 30, traveling through north Indiana. At the time of the truck accident the semi was pulling two trailers filled with barrels containing flammable resin and another unspecified corrosive material.

It had recently snowed in the region of the truck accident and many roads were slippery and coated with black ice, a form of ice that is almost invisible on pavement.

The truck driver had reached Kosciusko County at about 2:00 a.m. Thursday morning, still driving west on US 30. He was just past the juncture with Indiana Highway 13 when the truck accident occurred. According to police, the semi tandem trailer skidded on the icy highway and the driver, Mr. David, fought to keep the 18 wheeler under control. The semi trailer slid into the highway’s grassy center median and flipped over onto its right side, both trailer sections overturning.

No other persons or vehicles were involved in the truck accident.

The truck driver was momentarily trapped in the cab section of the overturned semi trailer. The Pierceton Fire Department rushed to the scene and helped the driver. Once he was out of the cab Wayne Township paramedics treated and stabilized the driver at the site. Mr. David was then transported by ambulance to Kosciusko Community Hospital in nearby Warsaw, IN. The driver suffered unspecified minor injuries in the truck accident. He was listed in stable condition and is expected to recover.

The force of the truck accident broke open the two trailer sections of the semi. The entire load of 31 barrels spilled into the eastbound lanes of US 30. Officials with the Kosciusko County Emergency Management Department quickly responded to the crash site. They determined that the load of resin and corrosive material posed a threat due to its flammability and shut down the highway in both directions. Indiana EPA hazmat specialists were also called to the scene to help clean-up the potentially dangerous materials.

Several semi trailers were brought to the truck accident site and crews used forklifts to transfer the 31 barrels to the other tractor trailers. It is believed that most of the barrels remained intact and little of the potentially hazardous materials spilled out onto the roadway. When the crews finished loading the barrels, a heavy wrecker got the tandem trailers back upright and hauled off the traffic lanes.

The highway was final cleared and reopened for traffic shortly after 9:00 a.m. Thursday morning, roughly seven hours after the single vehicle truck accident.

The Kosciusko County Sheriff’s Department was also at the crash site to assist in rerouting motorists around the overturned tandem trailers and onto secondary roads. Police have not yet issued an official statement but it is believed the truck accident was caused by the icy road conditions.

The truck driver was not charged in the truck accident.

On Wednesday morning 67 year old Bennie R. Nelson of Evansville, IN was driving a blue semi trailer truck south on U.S. Highway 41. The semi driver was traveling through Vanderburgh County at the time of the truck accident but his destination was not released. The trailer had the logo for NYK Logistics & Megacarrier, an international transportation company with US corporate offices in Secaucus, New Jersey.

At the same time, 70 year old Raymond Phillips of Waterloo, Illinois was driving a 2003 Chevrolet pick-up east on Boonville-New Harmony Road, also traveling through Vanderburgh County. There was one passenger in the pick-up at the time of the truck accident, 39 year old Timothy Phillips, also of Waterloo. It is not known if the two men are related.

The truck accident occurred a few moments after 10:00 a.m. Wednesday morning as the semi and the pick-up truck both approached the intersection of U.S. 41 and Boonville-New Harmony Rd. According to reports, when the eastbound Chevy pick-up reached the intersection its traffic signal was red. Mr. Phillips allegedly stopped for a moment before pulling out onto southbound US 41, directly in the path of the semi tractor trailer driven by Mr. Nelson. The truck driver did not have the time or space to avoid a collision and slammed full force into the Chevy pick-up.

The impact of the truck accident sent the Chevy pick-up truck out of control and it skidded sideways across the highway median and northbound lanes of US 41 before it went off the road and crashed into a chain fence. The pick-up finally came to a stop at the edge of the parking lot for a CVS Drugstore, trapping one of the men inside the cab section. The semi tractor trailer appeared to jackknife, and came to a rest in the northbound traffic lanes of US 41.

No other vehicles were involved in the truck accident.

A rescue team from the Scott Township Fire Department rushed to the scene of the two vehicle crash. The team had to use tools to reach the driver trapped in the Chevy pick-up. After cutting off the roof of the pick-up the crew extricated Mr. Raymond Phillips. Paramedics treated and stabilized the men at the truck accident site. The two were then transported by ambulances to nearby Deaconess Hospital in Evansville. As of late Wednesday both men were listed in critical condition, the driver fighting for his life.

The driver of the semi trailer, Mr. Nelson, was not injured in the truck accident. He was also taken to a nearby hospital where he underwent a routine blood toxicology test. The test is required and standard procedure whenever a commercial vehicle is involved in an accident. The police do not suspect drugs or alcohol were factors in the crash at this time.

Vanderburgh County Sheriff officers took control of the crash site and shut down a portion of southbound US 41. Vehicles were diverted around the truck accident site while emergency personnel helped the victims. Once the men were on their way to the hospital, highway crews came to the site to remove the destroyed Chevy pick-up and other debris from the traffic lanes. Sheriff Officers reopened the southbound lanes of US 41 shortly before 12 noon Wednesday.

Authorities investigating the truck accident talked to the truck driver and witnesses to the crash. Several people support the claim that the pick-up driver momentarily stopped at the red light before going through it, and driving into the path of the southbound tractor trailer. At this point it does not appear that the semi driver will be charged in the incident.

The truck accident remains under investigation.

At about 11:15 a.m. Thursday morning the truck driver was traveling in the vicinity of Remington, IN, his destination at the time of the truck accident unknown. According to police, Mr. Ikar was in the right southbound lane of I-65 when he realized that he had missed his exit. The truck driver allegedly stopped the tractor trailer, put it in reverse, and began backing up on the interstate.

The driver of a second Freightliner truck came up from behind and crashed into the back trailer section of Mr. Ikar’s semi. The collision happened near the interstate’s Mile Marker 205, near the juncture with U.S. Highway 24/231.

The driver of the second semi likely did not realize the first tractor trailer was backing up and may have hit the truck at full speed. The second driver suffered severe blunt force trauma in the truck accident and was killed in the crash. Police have identified him as 44 year old James Jones of Indianapolis, IN. The semi he was driving had the logo for Kroger Foods on the side of the trailer, a grocery store chain based in Cincinnati, Ohio. Mr. Jones was carrying a cargo of refrigerated foods for the grocery chain.

The force of the truck accident broke open the trailer section of Mr. Ikar’s semi and an unknown amount of rubber mats and other materials spilled out onto the highway. A third truck driver traveling in the southbound lanes of I-65 was hit by some of the debris from the crash. This damaged the trailer section of his 2002 Freightliner but the truck driver was not hurt. He has been identified as 30 year old Donald Moore of St. Paul, IN.

Mr. Ikar and his passenger were both slightly hurt in the rear-end collision. They were treated and stabilized at the scene of the truck accident and then transported by ambulances to Jasper County Hospital. After some additional treatment both men were released from the hospital.

Indiana State Police officers investigating the fatal truck accident questioned the driver of the semi that set off the crash, Mr. Ikar, and his passenger, Mr. Ama. Officers determined that the semi driver was responsible for the collision and he was arrested and charged with Suspicion of Involuntary Manslaughter, which is a Class D Felony. He was booked and as of Friday was held at the Jasper County Jail in Rensselaer, IN.

The passenger, Mr. Ama, allegedly gave false or misleading information while he was being questioned about the truck accident. He was charged with Suspicion of Obstruction of Justice, a Class D Felony. He was also booked and was being held at the Jasper County Jail.

State Police shut down a portion of southbound I-65 after the deadly crash. Highway crews worked for several hours to clear the traffic lanes and tow the disabled vehicles from the site of the truck accident. All lanes of I-65 were reopened by Thursday evening.

The truck accident investigation is expected to last several more weeks. The semi driver could have his charges changed or additional citations brought based on the findings of the investigators. The driver may also face civil liabilities if the victim’s family files a lawsuit.

Late Friday morning 47 year old truck driver Tarrance Lee of South Bend, IN was southbound on US 31, driving a semi trailer truck through Hamilton County, IN. The driver was carrying a load of steel cables in the trailer section of his rig when the truck accident occurred. The driver is employed by Storm Trucking, based out of South Bend. His destination is not known at this time.

At about 11:40 a.m. on Friday the semi driver was on the north edge of the Indianapolis metro region and was approaching 236th Street in Bakers Corner. According to police, when Mr. Lee reached the juncture he allegedly ignored a red stop light and entered the intersection. The 18 wheeler slammed into a white mini van which was traveling east on 236th. The van had the right of way at the time of the truck accident.

The tractor trailer continued forward, pushing the white van until it was pinned up against a tree. The force of the broadside collision crushed the left side of the van which had three people inside. David Clouser, 65, of Carmel, IN, was behind the wheel of the minivan at the time of the truck accident. His two passengers were Michael Zimmer of London, Ohio and Jeffrey Pinn of Rochester, New York. Mr. Clouser worked as a recruiter for a trucking company. The two men were truck drivers who had just completed training and Mr. Clouser was transporting them to their semi trailers. Mr. Clouser had just announced his plans to retire the day before the crash.

Indianapolis police and rescue workers rushed to the scene of the two vehicle collision after receiving several 911 calls. Sadly, Mr. Clouser suffered fatal injuries in the truck accident and was already dead by the time help arrived at the crash site.  The three victims were trapped in the crumpled minivan and emergency personnel would have to cut through the metal to free the men. After almost two hours they were able to reach the two surviving semi drivers. They were treated and stabilized at the crash site and then medically airlifted in Flight for Life helicopters to Methodist Hospital in Indianapolis. Mr. Pinn has internal injuries and Mr. Zimmer has injuries to his leg. The severity of their conditions has not been not released.

Mr. Lee, the driver of the semi trailer, was not injured in the deadly truck accident. The front cab section of his 18 wheeler had some damage but the load of steel cables appeared to have stayed on the trailer. There were no reports of fuel spilling out at the crash site. The intersection would be closed for several hours while crews cleared the traffic lanes and removed the destroyed minivan. State and local police reconstructed the incident and talked to witnesses.

Police investigating the truck accident attempted to get the driver to submit to a required blood test and breathalyzer test, but the truck driver refused. Officials also found that the semi driver either falsified his log books or misrepresented them to the investigating officers. At that point Mr. Lee was placed under arrest and placed in handcuffs. He was taken from the site of the fatal collision and was being held at the Hamilton County Jail.

At this time the semi driver has been charged with Criminal Recklessness with a Motor Vehicle Causing Death and False Informing. According to an official Mr. Lee may have his truck driver’s license suspended for refusing to submit to the toxicology tests. Additional charges could also be brought pending the completion of the truck accident investigation.

Early Monday morning truck driver John Williams of Milwaukee, Wisconsin was driving a semi trailer truck through Hamilton County, IN. The logo on the cab section of the truck was Bay & Bay, a transportation and freight company based out of Rosemount, Minnesota. The cargo and destination of the semi is not known.

The tractor trailer was westbound on 116th traveling through the town of Fishers when the truck accident occurred. It was about 4:20 a.m. Monday morning when the truck driver reached the intersection of 116th and Allisonville Road and attempted to make a right, northbound, turn into a gas station. As the truck driver, Mr. Williams, pulled into the gas station he suddenly slammed into an electrical utility pole.

No other persons or vehicles were involved in the truck accident.

After the initial crash the truck driver tried to back his 18 wheeler rig out onto the street, reportedly not realizing that he had hit the pole. Mr. Williams told authorities that he “saw sparks” after the truck accident but did not know that his semi was pulling down the utility pole until he saw the wires and pole crashing down on his cab. The tractor trailer quickly became ensnarled in the power lines.

The semi driver’s first instinct after the truck accident was to get out of the cab but another man rushed up and warned Mr. Williams about the live electrical lines that surrounded the tractor trailer. The truck driver would have to stay inside the cab section until rescue workers could cut power to the live wires.

The utility pole brought down by the truck accident supplied power to approximately one thousand homes and businesses in the area. The neighborhood was plunged into darkness after the crash, the outage affecting a 3 mile radius.

Fishers Police and the Fire Department Rescue team rushed to the crashed semi truck. Some of the firefighters began working to free the truck driver from the cab section while police closed down 116th St for about a block and diverted traffic around the truck accident scene.

The utility company sent a crew to the site to work on repairing the downed lines and restoring electricity to the neighborhood. They were able to cut the power on the lines surrounding the tractor trailer and the driver was finally extricated at around 6:00 a.m., almost 90 minutes after the truck accident occurred. Fortunately he was not injured in the incident.

The utility company’s crew continued to repair the power lines and electricity was restored for some residents and businesses at 7:30 a.m. Monday morning. Officials said it would take several more hours to repair all the lines and clear the truck accident scene. They expected the power to be completely restored by Monday evening. Some businesses used their own generators to provide electricity while other businesses were forced to close for the day.

At this time the truck driver has not been cited and it is not known if police plan on charging Mr. Williams in the future. There is not any information of the financial cost of the incident and whether the trucking company will be held responsible.

The truck accident remains under investigation.

Early Friday morning truck driver Timothy Clark of Franklin, IN was driving a semi tanker truck through Vigo County, IN. The driver had driven through Indianapolis earlier but the destination of the fuel tanker and the company that owns the vehicle is not known. It is also not known if the tanker section of the truck was filled with fuel at the time of the truck accident.

At about 7:30 a.m. the semi truck reached the city of Terre Haute, IN and was driving near the intersection of Fruitridge and Maple Avenues when the truck accident occurred. According to police the tanker truck was north on Fruitridge Ave. when he came up on an SUV that was stopped for a red light at the intersection with Maple Ave. The tanker truck driver didn’t begin to slow down in time and plowed into the back of the smaller vehicle.

The driver would later tell authorities investigating the truck accident that he had been feeling drowsy as he drove from Indianapolis and Terre Haute and had fallen asleep several times.

The force of the rear end collision shoved the SUV forward in the road for about 500 feet before finally stopping across the intersection of Fruitridge and Maple Avenues. The area of the truck accident is next to Terre Haute North Vigo High School. Fortunately the students and faculty of the school were not injured in the crash and school officials were able to reroute school traffic to another entrance off Maple Avenue.

The unidentified driver of the SUV was trapped inside his vehicle which had been crushed in the rear-end collision. Rescue teams responding to the truck accident had to use a variety of tools to cut through the metal of the SUV to reach the victim. Once the driver was free he was stabilized at the scene before being transported by medical helicopter to a hospital in Indianapolis.

Authorities have not released the name or condition of the SUV driver but it is believed the person was seriously hurt and received life threatening injuries.

The driver of the tanker truck, Mr. Clark, was not believed to be injured in the crash. However police who spoke to the truck driver apparently believed he was under the influence of a substance and sent him to the hospital for testing. He was given a toxicology drug screen at an unnamed hospital in Terre Haute. The tests returned positive results that the driver was using cocaine at the time of the truck accident.

Police officials also reported that they had received several calls about a semi tanker truck driving recklessly on Indiana State Route 46 just before Friday morning’s truck accident. They have not officially confirmed that the two incidents involved the same semi tanker truck.

The tanker truck driver was arrested by Terre Haute Police and detained at the Vigo County Jail. He has been charged with Driving While Intoxicated, Reckless Driving, and Criminal Recklessness Resulting in Great Bodily Harm. Additional charges could be brought against Mr. Clark pending the completion of the truck accident investigation. The chargers could also be modified if the condition of the unidentified victim becomes more serious.

Highway crews worked for several hours to clear the demolished SUV and other debris from the roadway. The intersection of Maple and Fruitridge was finally cleared and reopened for traffic a few hours after the truck accident.

Early Saturday morning, on October 23, 57 year old Oleh Pyvovarov of Milwaukee, Wisconsin was driving a semi trailer truck east on the Indiana Toll Road, also known as Interstate 80/90. The truck driver had a cargo of bagged apples and other produce in the trailer section of his semi. It is not known if he was the owner/operator or working for someone else. He may have been taken the produce back to Wisconsin but this has not been confirmed.

The truck accident happened at around 5:30 a.m. Saturday morning as Mr. Pyvovarov drove through St. Joseph County, IN. The produce filled tractor trailer was traveling near the toll road exit for Notre Dame University, South Bend at Mile Marker 77 when he came up upon an eastbound 2008 Freightliner semi trailer moving much slower than Mr. Pyvovarov’s 18 wheeler truck. The Freightliner was driven by 48 year old Jorge Infante of San Pablo, California and had one passenger, 62 year old Phillip Pascoe of Upland, CA. Mr. Infante was also carrying produce in the trailer section of his semi.

The Wisconsin driver was unable to slow down or maneuver around the second rig and slammed into the back section of the Freightliner. The force of the rear-end truck accident punctured a fuel tank and Mr. Pyvovarov’s semi truck exploded into flames. According to witnesses the flames shot about fifty feet into the air and created toxic plumes of black smoke. The trailer sections of both semis broke open and bags of apples and other produce spilled out onto the toll road.

Emergency teams rushed to the site of the truck accident. While firefighters battled the blaze on the semi paramedics tended to the drivers. Unfortunately, Mr. Pyvovarov was fatally injured in the crash and died at the scene.

The truck driver of the tractor trailer that was rear-ended, Mr. Infante, and his passenger were not injured in the truck accident.

The deadly collision closed down the eastbound lanes of the toll road and Indiana State Police were diverting vehicles around the crash site for most of the day. Once the truck fire was extinguished highway crews cleaned the debris off the road and transferred the produce to a second truck. It is not known if the produce will be destroyed. A heavy duty tow truck hauled the burnt wreckage of the semi off the highway.

Traffic on the toll road was still reduced to one lane at 6:00 p.m. Saturday evening, more than twelve hours after the truck accident. Officials expected all lanes to be open by 9:00 p.m.

Officials with the Indiana State Police are reconstructing the truck accident, trying to determine what caused the fatal crash. They have not confirmed if the driver of the front semi truck was slowing down to exit the Indiana Toll Road at the South Bend exit, if he had been driving under the speed limit for a period of time, or if he had suddenly reduced his speed.

A spokesperson with the department also stated that they did not find any tread marks on the highway during their investigation, which seems to indicate the neither truck driver was applying their brakes prior to the truck accident.

At this time no charges have been brought and the officials are continuing to investigate the truck accident.

On Sunday morning 18 year old Truc Huynh of Lafayette was driving a black 2001 Saab 93 south on County Road 975 East, traveling through Tippecanoe County, IN. There was one passenger in the car, 18 year old Ryan M. Lockwood, also of Lafayette. The two young men were freshmen at Purdue University, attending the Lafayette campus. Mr. Huynh was majoring in Chemistry and Mr. Lockwood was majoring in Computer Graphics Technology.

At the same time 56 year old truck driver Jeffrey B. Martin of Arcadia, IN was driving a 2000 Freightliner semi trailer truck east on County Road 28. Mr. Martin was carrying a cargo of corn in the trailer section of the truck at the time of the incident.

The truck accident occurred at about 10:50 a.m. Sunday morning as both vehicles were driving through the area near the town of Clarks Hill. According to officials, as the Saab approached the juncture of State Road 28 and County Road 975 East the driver, Mr. Huynh, failed to come to a stop and pulled into the intersection, directly into the path of the oncoming semi truck.

The truck driver, Mr. Martin, saw the car pull into the path of his 18 wheeler truck but was unable to slow down or maneuver out of the way and the trailer truck slammed into the Saab. Officials investigating the truck accident confirmed that the semi hit the car broadside, delivering extensive damage to the smaller vehicle.

The force of the truck accident pushed the black Saab off the road and into a nearby field running along the south side of Indiana SR 28.

Tippecanoe County firefighters and emergency personnel responded to the truck accident and attempted to help the victims. Unfortunately the two Purdue University students suffered fatal injuries in the collision and were pronounced dead at the scene.

Mr. Martin, the driver of the semi, sustained minor injuries in the truck accident. He was treated for injuries to his knee at the scene and did not require hospitalization.

Traffic was rerouted around the intersection for several hours after the truck accident. Once the emergency teams had completed their work highway crews came in to remove the wreckage of the Saab car.

The truck accident is being investigated by the Indiana State Police Motor Carrier Division. They have determined that both students and the truck driver were wearing their seat belts. There is a possibility that the Saab driver entered the intersection in the path of the semi because he had fallen asleep while driving. Authorities are also checking whether the use of a cell phone or other device led to the crash. They do not believe drugs or alcohol were factors in the deadly collision.

No charges have been brought and the truck accident remains under investigation.
