Tampa is located in Hillsborough County, Florida on the coast of the Gulf of Mexico in the western part of the state about 201 miles southwest of Jacksonville, 36 miles southwest of Lakeland and about 23 miles east of Clearwater.

The victim was identified as Melvin James Rolfe, 39 years old from Tampa. The accident occurred shortly after 6:00 a.m. on Thursday while Rolfe was traveling southbound on U.S. 41 near South 34th Ave.

A semi trailer was making a left turn onto U.S. 41 northbound from eastbound 34th Ave South. The tractor trailer stopped in the middle of the highway, blocking all southbound lanes of U.S. 41 to yield to traffic in the northbound lanes.

Rolfe was in a Ford Explorer traveling south on U.S. 41 in the center lane. He braked and swerved to the right in order to avoid hitting the big rig but was unable to and crashed into the truck’s undercarriage. His vehicle became pinned under the 18 wheeler.

The truck driver, Rafael Meireles, 41 years old, also from Tampa, was not injured in the accident.

The accident is currently under investigation by the Florida Highway Patrol.

The accident happened at State Road 64 and 64th Street Court East around 4:15 Thursday afternoon. The Florida Highway patrol told the media that the bus collided with the back of the tractor trailer.

Twenty two students and the bus driver were transported to three area hospitals with minor injuries.

Reports say that the driver of the tractor trailer did not appear to be injured.

Students, who call their school bus driver ‘Mr. Charles,’ told Florida state troopers that he was distracted while driving down SR 64 East in Bradenton.

The children say the driver was facing the children, not the road, and chastising at least one student, threatening to write him up. He had the referral, and was looking back,” said 11-year old student Elijah Moore.

“He turned his head around to give someone a referral,” said student Mario Perez.

He was so distracted, they say, that students braced for the tractor trailer in front of them before the bus driver did.

“I instantly looked, and I saw the semi-truck,” said student Darnie Pascal.

When the tractor-trailer didn’t stop their school bus, a good Samaritan tried to put the brakes on. The bus came to a stop 500 yards from the original accident scene.

The accident remains under investigation at this time.

The accident occurred when Wright pulled out of a driveway of a local business heading east on County Road 218 toward Middleburg and into the path of the oncoming logging truck that was going the opposite way. The accident happened about 10:15 am on Thursday.

The semi truck crashed with the pickup truck and then overturned and spilled logs all over the roadway. The big rig driver, 36 year-old Mark Lamont Webb, from Crescent City, FL, was taken to a nearby hospital with minor injuries.

According to the Florida Highway Patrol, Wright did not notice the log truck when he pulled out into its path on County Road 218.

The incident remains under investigation at this time.

Jose Veciana, 89, who showed early signs of Alzheimer’s disease and his daughter Jeanette Aguiar, 57, were rushed by Paramedics to St. Joseph’s Hospital with life-threatening injuries. The father died.

The daughter, 57, remained in critical condition Wednesday afternoon. Her mother, Norma Williams, doesn’t expect Aguiar to make it.

“It doesn’t look good,” Williams said, crying. “We just keep praying and praying.”

She is no longer married to Veciana. Her daughter recently took him in.

The new arrangement was difficult for Aguiar, who suffers from back problems so severe she hasn’t been able to work in five years. Still, she wanted to take care of her father, a retired developer, said Williams, 86.

They lived together in a home near the Lowry Park Zoo, not far from the crash site.

And though Aguiar had to leave her job with Coca-Cola because of her back, she still worked from home making jewelry, her mother said.

Family members were with Aguiar through the night. She has three brothers, one sister and an adult son who lives in Louisiana, Williams said.

Yidier Morales, 42, the FedEx driver, and his passenger, Patrick McHayle, 38, suffered minor injuries in the crash.

Tampa police are still investigating the collision and ask that witnesses call the department at (813) 231-6130.

Early Monday morning 52 year old Wayne Earle of Palm Bay, FL was driving a semi trailer truck south on Boggy Creek Road. The driver was carrying a cargo for the U.S. Post Office at the time of the truck accident.

At the same time, 50 year old Rockland Steel was driving a full size Disney Magical Express tour bus north on Florida State Route 417. The bus was carrying 50 passengers, reported to be a group from Yuba City, California who were in town to attend a cheerleading competition at the Disney Resort. The group had been in town since last Thursday and was on its way to the airport for the return flight home at the time of the truck accident. The tour buses are owned and operated by the Mears Transportation Group, based in Orlando.

The truck accident occurred just before 5:00 a.m. Monday morning as the U.S. Post Office tractor trailer was approaching the juncture with the off ramp from northbound Florida State Route 417. According to reports, The U.S. Postal semi and the tour bus both reached the intersection of Boggy Creek Road and FL 417 at the same time and crashed into each other.

Orlando police and fire rescue personnel rushed to the scene of the two vehicle crash. Five people on the tour bus, including the bus driver, were injured in the truck accident. They were treated and stabilized at the scene and then transported to Orlando Regional Medical Center. The injuries are described as non-life threatening and all are expected to recover. A second tour bus was brought to the crash site to pick up the remaining 46 passengers and take them to the airport where they caught their flight home to California.

The driver of the U.S. Post Office tractor trailer was not injured in the truck accident.

The cab section of the U.S. Postal tractor trailer sustained significant damage in the truck accident and was partially crushed. The tour bus had some front end damage from the collision. The force of the crash also punctured at least one gas tank and about 150 gallons of diesel fuel spilled out into the intersection.

Police shut down the roads around the truck accident site while crews worked to cleanup the spilled fuel and remove the disabled vehicles. Boggy Creek Road was closed in both directions and the off ramp from FL 417 was also shut down while the crews cleared the traffic lanes. The closures came at the start of the Monday morning commute, creating major delays at the busy intersection.

All roads in the area were reopened about 4 hours after the truck accident.

Officials are currently investigating the collision between the two commercial vehicles. The intersection is controlled by a traffic signal and one of the large vehicles went through a red light, but police have not yet determined which driver was at fault. Police investigating the truck accident said the two drivers have given conflicting accounts of the crash. The semi trailer driver, Mr. Earle, stated that he had a flashing yellow light when he drove into the intersection, while the tour bus driver, Mr. Steel, stated that he had a green light. Florida Highway Patrol is consulting with traffic engineers to determine the cause of the crash.

Charges will likely be brought against one of the drivers once the investigation of the truck accident is completed.

The injured crewmen were photographer Jim Hockett and reporter Jackie Barron. The accident happened around 3:30 p.m. on Wednesday as Barron and Hockett were returning from an assignment in Venice.
Barron and Hockett were taken to Sarasota Memorial Hospital for observation and where they were treated for minor injuries after the crash.

Don North, Channel 8 news director said, “Both of them are in pretty good shape.”

The two had finished an assignment in Venice and were headed to a vehicle rollover about 3:30 p.m., said Florida Highway Patrol Trooper Kenn Watson.

As they headed north in the center lane of Interstate 75 in Sarasota the rear left tire on the News Channel 8 van blew, Watson said. Hockett, who was driving, began to lose control because the van is top heavy with equipment, Watson said.

The van rotated clockwise and overturned. It came to a final stop in the outside lane, Watson said.

Barron was able to get out of the van, but Hockett was trapped inside. Rescue crews assisted in getting him out, North said.

“It took awhile to get Jim out of the vehicle,” North said.

According to News Channel 8 chief engineer, Mark Schaefer, the only serious damage was to the van, which was estimated at $125,000 to $150,000.

Early Wednesday morning Roger Douglas of Lake Butler, FL was driving a semi trailer truck south on Interstate 95, traveling through Palm Beach County. The semi truck was carrying a load of PVC pipes and the truck’s cab had the logo of Silco Carthage, Inc.

Shortly before 3:00 a.m. Hudson Furtado of Orlando, FL was driving a 2005 Mazda sedan southbound on the interstate when he suddenly lost control of his vehicle for unknown reasons in the area near 6th Avenue South. The Mazda crossed over the highway and directly into the path of the semi trailer truck. The truck driver attempted to maneuver to avoid an accident but could not and slammed into the Mazda sedan.

After hitting the Mazda the tractor trailer crashed into a concrete barrier at the side of the highway and overturned. The force of the truck accident broke open the cargo section and PVC piping spilled all over the interstate. The pipes were strewn across the highway creating an immediate hazard for other drivers on the roadway. At least one witness reported that his windshield was cracked from the piping and debris that flew through the air directly after the collision. Other vehicles drove into the PVC pipes that suddenly fell into the traffic lanes.

At this time it is not known exactly how many other persons or vehicles were ultimately affected by the truck accident.

The impact of the truck accident also punctured the semi’s gas tank and the 18 wheeler exploded into flames. The fire quickly engulfed the trailer truck and local firefighters rushed to the scene to battle the blaze. According to the head of the Palm Beach County Fire Rescue, 12 separate firefighting units responded to the burning semi truck, sending about 25 firefighters. It would take several hours for the crews to finally extinguish the truck fire.

Emergency medical personnel also responded to the truck accident site. Fortunately the driver of the Mazda and the truck driver were apparently both able to escape their vehicles and were not seriously injured.

Florida Highway Patrol shut down a section of I-95 surrounding the truck accident site. Southbound traffic quickly became backed up as investigators and various crews worked at the scene. Highway crews worked to gather the PVC piping and other debris that was scattered across the interstate. Once the fire on the trailer truck was extinguished a heavy duty tow truck was brought in to remove the burnt wreckage of the semi from the highway.

The truck accident caused extensive damage to both vehicles with one official estimating about $60,000 of damage to the semi trailer and $10,000 to the Mazda sedan.

The traffic lanes on I-95 were finally reopened shortly before 9:00 a.m. Wednesday morning, about 5 hours after the truck accident occurred.

Authorities investigating the truck accident found that the driver of the Mazda, Mr. Furtado, was probably at fault.

According to reports he will likely be charged with Failure to Use Due Care pending the completion of the truck accident investigation.

The semi cab of the truck burst into flames, almost completely destroying the SUV.

At around 2:00 a.m. Friday morning 39 year old Craig T. Roy from the Jacksonville area was driving a semi trailer truck west on Interstate 4 through Orange County, FL. One unidentified passenger rode along in the cab of the semi. The semi truck belonged to UPS and was pulling two trailers loaded with a full cargo of mail parcels and shipping packages. The corporate offices for UPS (United Parcel Services) are in Atlanta, Georgia.

The UPS tractor trailer was traveling in one of the center lanes of the interstate near the city of Fairview Shores when the driver suddenly lost control of the 18 wheeler in the area between Lee Road and Princeton Street. According to police, the truck driver Mr. Roy told them he was experiencing some kind of mechanical failure that caused him to lose control of the truck.

The UPS tandem trailer jackknifed and slammed into the highway median. The trailer truck hit a Ford Edge SUV driven by Michael Shaver and dragged the SUV along the wall of the interstate before finally coming to a rest. The semi cab of the truck then burst into flames. The SUV was almost completely destroyed in the crash.

Several passing drivers including a doctor saw the truck accident and stopped to help the victims. Mr. Roy and his passenger were able to crawl out of the burning cab of the semi truck and escape serious injuries. The driver of the Ford SUV, Mr. Shaver, crawled through the back window of his vehicle.

The doctor who stopped was surprised that the three people sustained only minor injuries in the truck accident. Emergency personnel treated and stabilized Mr. Shaver and the passenger in the semi and then transported them to a nearby hospital. The semi driver did not require hospitalization.

The impact of the truck accident damaged at least one fuel tank and about 75 gallons of diesel fuel spilled out onto the interstate, feeding the flames of the truck fire. Local firefighters were soon at the scene and battled the flames.

The force of the truck accident also broke open the UPS trailers and mail packages were strewn all over the highway.

Florida Highway Patrol shut down the westbound lanes of I-4 at Fairbanks Avenue while the EMTs and firefighters worked. Highway crews were brought to the truck accident site to get the wreckage of the UPS trailer truck and Ford Edge SUV off the roadway. Others teams worked to clear the parcels and packages out of the traffic lanes and a Haz-mat crew cleaned up the remains of the spilled diesel fuel.

It would take about five hours to clean up the truck accident scene, causing major delays for the Friday morning commute.

Authorities conducted a reconstruction of the truck accident to determine the cause of the fiery crash. The Florida State Commercial Trucking Investigation Unit is expected to complete a full inspection of the UPS trailer truck since the truck driver claimed mechanical problems led to the crash.

No charges have been brought at this time and the truck accident remains under investigation.

Shortly before 8:00 a.m. Thursday morning 61 year old Richard Wilson of Venice, FL was driving a semi trailer truck south on Interstate 75 in Sarasota County, FL. As Mr. Wilson was traveling near the River Road Exchange near mile marker 191 he reportedly lost control of his 18 wheeler truck.

The tractor trailer truck went off the interstate, hitting a box by the side of the road. The semi truck then crashed into a highway guardrail that apparently broke off and became stuck on the truck’s lower section. The trailer truck dragged the section of guardrail almost a quarter of a mile down the interstate. The guardrail section created sparks as it was pulled along the road surface and these sparks ignited one of the semi truck’s fuel tanks. An unknown quantity of diesel fuel spilled out onto the interstate from the damaged fuel tank.

When the semi trailer finally came to a rest along the interstate, the driver, Mr. Wilson, was able to crawl out of the cab and escape the burning 18 wheeler. Emergency personnel responded to the truck accident scene. EMTs checked out the truck driver, who was uninjured and did not require hospitalization.

The fire engulfed the trailer truck and local firefighters fought the blazing rig which was completely destroyed and left a burnt shell.

A Haz-Mat team cleaned up and contained the diesel fuel spill while highway crews cleared the debris and worked to remove the wreckage of the demolished semi trailer truck.

Florida Highway Patrol shut down the interstate for about an hour around the truck accident scene while rescue personnel worked and authorities reconstructed the incident. They later opened up some of the interstate while diverting traffic between Jacaranda Avenue and River Road.

Authorities reconstructed the truck accident but have not determined why the semi trailer went out of control and crashed.

No charges or citations have been made at this time and the truck accident remains under investigation.

The cab of the semi trailer dangled over the edge above the Intracoastal Waterways.

Shortly before 11:30 a.m. Tuesday morning 32 year old George Gonzales was driving a semi trailer truck west on the Cortez Bridge. Mr. Gonzales was working for Cummings Foods based in Dover, FL and was making a delivery to the Publix Grocery Store in Anna Maria Island.

As the semi tractor truck crossed the bridge between Cortez and Bradenton Beach he encountered an eastbound Volkswagen driven by 43 year old Jonathan Quick of Bradenton Beach, FL.

As the two vehicles neared each other the truck driver lost control of his semi trailer truck for unknown reasons and crossed into the eastbound lanes of traffic. He was unable to maneuver to avoid a collision and slammed into the VW car.

After hitting the car the 18 wheeler truck continued forward and was finally stopped when it became stuck on the VW. The cab of the semi trailer dangled over the edge above the Intracoastal Waterways. According to officials the semi trailer truck might have gone over the edge and plunged into the water if it had not been stopped by the VW car. The trailer and fuel tanks on the semi truck remained intact and no cargo or diesel fuel were spilled.

Manatee County Safety Department officers and emergency personnel responded to the scene of the truck accident. Both drivers suffered minor injuries in the collision and were treated at the scene.

Officers shut down a section of the Cortez Bridge in both directions surrounding the truck accident site while rescue personnel worked and investigators reconstructed the collision. Traffic was stopped and drivers were directed to take the Manatee Avenue Bridge. A wrecker was brought in to remove the disabled semi truck from the bridge edge and remove it from the traffic lanes. All lanes on the bridge were reopened shortly after 1:30 p.m. Tuesday afternoon.

Officers investigating the truck accident have not determined why the truck driver, Mr. Gonzales, lost control of his rig and crossed into the eastbound lanes of traffic. It is not known if drugs or alcohol are factors in the collision.

No charges have been brought at this time and the truck accident remains under investigation.
